You are halfway through a contrast-enhanced CT when you notice it: slight swelling at the IV site. Your patient mentions a burning sensation. Your heart rate ticks up—but your training kicks in even faster.

Contrast extravasation happens. Even with perfect technique, even on your best day. The literature tells us it occurs in 0.1 to 0.9 percent of power injections. That translates to several events per year in a busy imaging department.

This is your five-minute refresher on exactly what to do when it happens—so you can respond with confidence, document thoroughly, and ensure the best outcome for your patient.

Immediate Response: The First 60 Seconds

The moment you suspect CT contrast extravasation, your priority list is short and clear.

Stop the injection immediately. Do not wait to confirm the volume or severity. If your patient reports pain, burning, or tightness—or if you see swelling, coolness, or blanching at the site—stop the power injector right away.

Next, disconnect the IV line from the injector but leave the catheter in place temporarily. This allows you to aspirate any contrast still sitting in the subcutaneous tissue. Some facilities use a small syringe to gently pull back 3 to 5 mL; others skip this step depending on protocol. Know your department’s standard.

Remove the IV catheter only after attempting aspiration. Apply a sterile dressing.

Assessment and Documentation

Once the injection is stopped and the IV is managed, shift into assessment mode. This is where imaging safety and radiology response protocol intersect with legal protection for you and your facility.

Document the following details in real time:

  • Exact time the extravasation was identified
  • Volume of contrast extravasated (estimate based on injector readout and visual assessment)
  • Type and concentration of contrast agent used
  • Anatomical location and appearance of the site (swelling size, skin color, temperature)
  • Patient’s subjective complaints: pain level, burning, tingling, numbness
  • Immediate interventions performed (aspiration, elevation, ice or warm compress per protocol)

Most departments require you to photograph the site if possible, with patient consent. A quick image provides objective evidence of the initial presentation and supports later comparison.

Notify the radiologist and the ordering physician. In many hospitals, you will also complete an incident report or patient safety event form. Do this before you leave your shift—memory fades, and details matter.

Patient Care and Comfort Measures

After documentation, return your focus to the patient. Extravasation is uncomfortable and often frightening, especially for someone who has never experienced it.

Elevate the affected limb above the level of the heart. This is the single most effective intervention for reducing swelling and promoting reabsorption of the extravasated contrast. Encourage the patient to keep the arm elevated for the next several hours—ideally 24 hours intermittently.

Apply a cold compress for the first hour to reduce swelling and provide comfort. Some protocols call for warm compresses after 24 hours to promote circulation and contrast reabsorption. Verify your facility’s standard; practices vary.

Explain what happened in plain language. Most patients appreciate honesty: “The IV contrast leaked into the tissue around your vein instead of staying inside the vein. This can cause swelling and discomfort, but it almost always resolves on its own within a few days.”

Provide written discharge instructions. These should include:

  • Elevation guidelines
  • Signs of complications to watch for (increasing pain, blistering, skin discoloration, numbness)
  • When and whom to call if symptoms worsen
  • Follow-up timeline (typically 24 and 48 hours)

Follow-Up and Complication Monitoring

Most contrast extravasations resolve without intervention. But a small percentage—especially large-volume events or those involving certain high-osmolality agents—can lead to complications.

Serious complications are rare but include compartment syndrome, skin necrosis, and nerve damage. These are more likely when extravasated volume exceeds 100 mL or when the site is near a compartment boundary (forearm, lower leg).

Your CT tech refresher is not complete without knowing the red flags that warrant surgical consultation:

  • Severe, escalating pain disproportionate to physical findings
  • Skin blistering or ulceration
  • Altered sensation or motor function distal to the site
  • Tense, firm swelling suggesting compartment syndrome

Standard follow-up is a phone call or clinic visit at 24 hours and again at 48 hours. Many departments assign this to a nurse or radiology manager, but as the tech who was present, you may be asked to contribute observations or clarify timeline details.

Prevention Strategies Worth Revisiting

Even though extravasation is never entirely preventable, a few strategies reduce risk.

Choose the largest, most proximal vein available. Antecubital veins are ideal for power injection. Hand and wrist veins are higher risk due to smaller caliber and less surrounding tissue.

Check IV patency before connecting the power injector. A saline test bolus—10 to 20 mL pushed by hand—lets you confirm flow and watch for swelling before high-pressure injection begins.

Use proper injection parameters. Flow rates above 5 mL/sec increase mechanical stress on the catheter and vein. Match your rate to the clinical indication and the patient’s venous access.

Stay in the room during injection whenever possible, or maintain direct visual contact through the window. Early detection—catching the extravasation in the first 10 or 20 mL—dramatically improves outcomes compared to discovering it after the full bolus has been delivered.

A Final Word on Confidence and Care

Contrast extravasation is one of those low-frequency, high-stress events that can rattle even experienced imaging professionals. But with a clear response protocol and calm, systematic action, you can manage it safely and professionally every time.

Your role as a CT tech extends beyond image acquisition. You are the patient’s advocate, the safety monitor, and the first responder when things do not go as planned. That is worth remembering on the ordinary days and the complicated ones alike.
